Sport Physical Form
Insurance Waiver

This form will need to be filled out in order to play any sport at Newman.  Please view, print, fill out, and return before or at your first practice.

Medical Consent Form

This form needs to be completed, signed, and turned in before any practice begins.

IHSA PERFORMANCE DRUG POLICY FORM

ALL ATHLETES AND THEIR PARENTS MUST SIGN THIS AND RETURN TO SCHOOL.  (only needs to be done once)

Concussion Inforamtion Signature Form

This form must be signed by both the student and parent and returned to school
